What, if any, are the additional hurdles that foreign grads must face to practive in the US? I'am confused that foreign grads worry about passing the USMLE and, perhaps, other exams to practice in the US; don't US grads also required have to pass USMLE to practice?

There is no longer any difference. We have to take (and pass) only the exact same exams as U.S. allopathic students. The only distinction is that we have to be certified through ECFMG, which can be a bit cumbersome but relatively painless process.
