I'm doing some research for my sister who is about to start 4th year. Naturally, I would like her to stay near me and do her residency at Mayo but she noticed Mayo-Scottsdale has only 8 openings for IM residents, which means it could be more competitive than Mount Sinai in NY. From what you have heard, which program has a better reputation particularly since she is interested in pursuing a GI fellowship. She thinks she might have an easier time matching at Sinai because they have around 40 spots. Her stats are as follows, reputable private medical school (ranked somewhere between 30-40), 233 Step I, good interviewing skills, middle of class rank

MCS is a community program, unlike Mt. Sinai. I've done two rotations at Mayo, one in the IM department, one in rad-onc. It is a fantastic institution, a true Mayo, but it does not have the reputation, history, or fellowship options that a true academic center has. Also, there is no urban experience, but I think there might be an HIV rotation down at Maricopa. The GI department is amazing.

Matching at Mt. Sinai would be very challenging - NYC is a desirable match location, and that is one of the top 5 or better programs in the city. You'd need strong boards and a pretty good class ranking, which she seems to have. But, there would be no guarantees, b/c it is a competitive program.

Matching at MCS would probably involve rotating there, making a good impression, and telling them how badly you want to go there. I'm not saying it's not competitive - it's almost entirely US Grads (mostly from U of A, Texas schools, and a scattering of others), but it is still a community program. They are going to 12 spots, but that will be a gradual change.

They've sent people to other institutions for GI, and I think they recently took one of their own, but they don't make a habit out of it (when I was considering GI, I asked faculty about it), and they also have many Mayo Rochester residents applying.

The comparison is pretty difficult to make. I've loved it here, but I'd rather do a fellowship here or work here as an attending than do a residency. It's not a broad enough experience for my taste - no county, no VA.

I'd suggest rotating here. It's a pain in the ass application, though - essay, board scores, letter of rec.

Thanks for the great answer. Yeah, she is experiencing that pain in the ass process as she is applying to do a rotation in Scottsdale as we speak. I'm being selfish because I would like to have family around but I think Sinai might be a better fit for her from what you described.
