29 year old white male was seen recently in our sleep center. The patient brought Ambien 10mg and also had Effexxor 75 and Provigil 200 (early am). 10 mg of Ambien was taken at 10:30, affter no sleep 5 mg at 11:30 and 5 mg at 12:30 (administered by the patient and without our involvement). Patient appears to be sleepy silent, arouses at 2:05 and takes one more 10 mg Ambian. After this point the patient fell asleep. Interesting finish, at around 4:10 am the patient picks up his cell phone and makes a couple of calls. Then at 4:53 am. he gets a call from his mother. All while appearntly asleep. In addition, the patient was suprised when he talked to his mother the next day and found out he had talked for over 6 minutes according to his cell phone and didn't remeber any part of picking up the phone to call or answering the call. They conversed, but the mother stated that something did seem odd or different. The patient was very calm aswering in full sentices and feedback to the mother but seemed less himself, possibly drugged up. Upon hearing the story of the conversation the patient was able to identify the words and things that were said as occuring, although in his mind he has the feeling that the was not real context for the memory, for example the words aren't associated with any real memory of sound of vision, he is just able to acknowledge that he can understand that her said everything his mom relayed about their late nite conversation. The patient was under the impresion that he went to sleep at 2:00 am and didn't wake until 5:30 am via the sleep techs.
Another curious thing about this patient is that he just had an uneventfull sleep study 3 weeks ago that concluded with 47 minutes of sleep time and presumptive diagnosis of mild obstructive sleep apnea. What is going on with this patient! I wish I had more of the sleep study data, but its just not available to me yet.
Oh one other factoid, he's had long standing insomnia due to anxiety and stress. Basic school and family issues.
