Common. Generally you will get a medical license during your residency (for the state you're in obviously). If you get a job in another state you just have to get a license there before you can work. I say "just" with a little chuckle because it's usually an annoying several month process but it is doable.

It is very common. You will apply to get your liscense for the new state well in advance of graduating.

Not every residency requires you to get a medical license. And state licenses are expensive to obtain and renew(unless your residency program reimburses you). So, if you're absolutely sure of the state you want to practice in, you should start applying for a license in the fall/spring of your final year of residency.
