My sense is that most cities with university affiliations that train ophthalmologists are busy but they continue to practice build by investing heavily in outreach and the usual PR stuff. If you have an interest in practicing in the midwest I would like to put in a plug for Marshfield Clinic, www.marshfieldclinic.org.
