With your step scores you will likely match somewhere. It will be hard to get into a university based academic program w/ your scores but a community program should be attainable. Apply to as many programs as you can some academic and most community. Casting your net wide will increase your chances of matching in IM. Also, if you are aiming for a competitive fellowship then you should shoot for an academic prog, so try to get involved w/ research w/ a faculty if you can or an externship. Either of those will significantly increase your chances of matching into a particularly program. Also, a solid step 3 will certainly help.

You may still have a chance at various university programs that are in smaller cities. Avoid the east/west coast university hospitals. If you're willing to live in some less popular cities/towns, you may have a very reasonable chance. Of course, this also depends on your grades/letters/etc.

Agree with adjkim,
People underestimate the importance of geography in the number of applicants (and thus the selectiveness) to get in to various residency programs. Any place in a big east/west coast city or Chicago will be harder to get in to. If you try to apply to U of Iowa or another Midwest or South university program, you may find they are more receptive than you think.

Have got an LOR from a foreign- born, american board certified interventional cardiologist. How much is that going to help?
Moreover, many thanks to you all for responding to my query.

A letter from a US board certified cardiologist will be helpful, particularly if he has worked with you clinically. If you only worked w/him doing research, it probably won't help as much. The letter will also help more at places where one of the faculty knows him...
