When is the best time to take an Ophth. elective? Why? Before or after IM/surgery ? How many Ophth. externship one should have at the least? As far as prepartion goes, what is the most important thing one should do before starting an Ophth. elective ?

You should take the ophthalmology elective at the end of the third year or beginning of your fourth year of medical school. This will allow time for you to obtain letters of recommendation from ophthalmology faculty before applying. To be competitive, you'll need outstanding letter(s) from faculty in the field.

One month of elective is usually enough. Some do 2 or 3. It's advised that you also arrange an elective at the program you're most interested. While this may not be possible, it is a huge bonus if you can arrange this away rotation.
