I am torn between these two as to which one I should rank higher. I am interested in doing a fairly competitive fellowship eventually and wanted to get others opinions regarding the reputation and training of these programs. Lahey is a smaller program and has an awesome PD. UMass seems to be more academic. Both are friendly and location is about the same. Any thoughts are appreciated.

I know nothing about Lahey but interviewed at UMass 3y ago and really liked the program (except for that whole Worcester thing). In general, if you want to get a competitive fellowship outside of your home institution, you will be better served at an academic center than at a community hospital w/ academic affiliation. There are of course many exceptions to this rule but it's a good place to start.
That said, if you're choosing between an academic program that you'd be miserable in for years vs a community program that you love, the latter may be better for you. Only you can make that choice.
UMass doesn't list their recent fellowship matches on their website. Lahey lists them for the last several years. Of the "competitive" fellowships (Cards and GI) they mostly match to Lahey with one going to U of C for cards and one to Kansas for cards. If you have to go to Duke for your cards fellowship, you'd be better served by the academic center.
