I am going to be starting a residency in IM in a few months. I am interested in your opinions on what might be the "best" current subspecialty of IM. I know that the word "best" is vague and not specific. Nevertheless, tell me how a subspecialty is "best" to you. I would really appreciate your answers, especially if you can give an opinion and support it well.

This question cannot be answered, because it totally depends on YOU and what you find to be BEST. Do you mean best hours (probably endocrine or rheum or allergy/immuno). Best money for least work (I would say allergy/immunology). Most money (? GI vs. cards). Most action (critical care or cards). Most procedures? (GI or cards). Most important to society (probably general IM, IMHO).

I'll take a stab at this. I'll give my opinion and I'll support it the best I can. I think you should do geriatrics. It is a great combination of absolute certainty of impending mortality mixed with very cool procedures like dobhoff tubes. The polypharmacy is riveting. Giving meds that are renally dosed is a skill that takes years to master. And the differential diagnosis for altered mental status given by a geriatritician makes even cardiologists bow their heads in respect. Nothing is more exhilarating than doing chest compressions a 98yr old whose family wants "every thing done". I think you'd be a perfect fit.

Welcome to Internal Medicine. Here you will find a cadre of independent thinkers who embrace change & truly think "outside of the box". (just google Barry Marshall & H pylori for an example)

Best specialty? Follow the numbers. The most competitive specialties make the most bank: GI & Cards.
