I've been hearing people say such things as "top tier" and "middle tier" IM programs...
Is there a list that ranks the programs? or is it more perception?

There are as many lists as there are people who care. None of them are particularly useful and it is, in general, a completely subjective argument. You can lose sleep over these "tiers" if you want but it's not really worth it.

agree with gutonc . . . too much mental masturbation about this topic in here
you need to ask yourself what you want to do, if you have any idea, and go from there, for instance some programs may be better than others for getting you into a certain competitive fellowship or in a location you want etc.

No list for ranking programs. There are regional biases - someone in the northeast may consider Yale to be a better IM program than UCLA, whereas someone from the west coast might think the reverse.
It seems that most academics consider MGH, BWH, JHU, and UCSF as the top four programs.
Other top programs would be UPenn, Duke, WashU, Mayo, etc.
Go to the place that will open up the greatest range of opportunities to you and where you feel most comfortable.
