Dr. Lyon has worked extensively in Haiti, most recently at the central hospital in Port-au-Prince. He describes how the recent cholera outbreak is particularly devastating for a nation still reeling and recovering from the catastrophic earthquake that it suffered in January 2010.

Well, cholera is from a lack of sanitation in Haiti. It could happen anywhere. Kala Azar is a parasite that happens to live primarily in third world countries where they are a) less able to treat it and b) less able to do anything to prevent it. But no, I don't think it has much if anything to do with the world being "smaller".
