You should use the USMLE Audio both ways
1. while doing chores and other tasks and
2. while sitting with a book open in front of you and taking notes. I did both.
You will find that when you listen to the audio while doing tasks, chores, or even going to sleep that you will retain information each time you go through the program. When I was in medical school that is exactly what I did. A strange thing that happened was that I could remember exactly what I was doing when I would remember a part of the audio. That is to say, if I listened to congestive heart failure while vacuuming the office. I would remember vacuuming the office while recalling factoids about Heart Failure.

Can I Learn While Asleep?

Sitting with a book open and using USMLE audio
Retention of USMLE audio material is indeed maximized when you are in a quiet place focusing only on the USMLE audio or the USMLE Audio along with a printed text and/or taking notes. The more engaged you are and the more senses you can stimulate the more you will retain. When the USMLE exams were getting closer, I would simply go to a quiet room and turn on the audio with or without a book/notes open to really focus on the audio.

DoctaJMa - Currently none of the USMLE Audio Reviews actually come with a book to follow. However, there is a PDF electronic copy of a Reference Manual for Gold Standard Step 1 and books are available for the USMLEHelp Audio reviews - sold separately by the publisher. In short, here's how the USMLE Audio Reviews break down:
USMLEHelp Audio USMLE Reviews
Each Apollo USMLE Audio Review that is based on a "USMLEHelp" book is an unabridged audiobook version of the textbook. The textbooks are not included with the audio (thus we name them "Audio" reviews or "MP3 Audio"). However the textbooks can be purchased from Amazon via any of the program listing pages found at Apollo's USMLE Audio Title List Page Here (http://apolloaudiobooks.com/page.php?id=75).
Gold Standard USMLE Audio Reviews - Step 1, Step 2, and Foundations of Medicine
Gold Standard Step 1 USMLE Audio - this does come with a 300 page "Reference Manual" in electronic PDF format included with the audio that follows the USMLE Audio review fairly closely. You can take notes in the margins and "reference" some of the hard-to-remember material. Also there are some essential diagrams like the brachial plexus. This is NOT a word-for-word transcription, but an outline format of the topics covered in the majority of the audio.
Gold Standard Step 2 USMLE Audio and Gold Standard Foundations of Medicine USMLE Audio - currently there is no text or book or PDF that accompanies these programs. At Apollo Audiobooks we are working to fix this deficiency by having real medical students transcribe all of the audio into a working transcript that can be used for future program updates or to create a text for students. So for now, these two programs are without printed material.

Students have reported using Apollo's USMLE Audio Reviews in both situations. However, most students prefer to get the programs early and use them throughout medical school to fill up their "down time" at the gym, commuting, walking the dog, et cetera. Those same students then focus more time on the audio reviews as the USMLE approaches.
I would equate students' use of the USMLE Step 1 and USMLE Step 2 Total Audio Packages, in some ways, to the way that First Aid for the USMLE Step 1 is used. As a resource, the sooner you are familiar with First Aid for the USMLE Step 1, the sooner you have a framework of knowledge upon which to build your USMLE Study program. While First Aid does not contain every bit of minutia that you will be tested upon in your classes, it contains many of the high-yield topics. Thus, if you go through First Aid with your classes - you will be more familiar with it and more prepared to use when "cramming" for the USMLE.
The same goes for Using Apollo's Gold Standard or Total Audio Packages. If you start listening to them casually, or when sitting to study, or when you just can't sleep - you grow more and more familiar with the material. This helps you with your classes and in prep for the USMLE. Thus you are more prepared to use the material in your USMLE "cramming" when the time comes.
In general, in medical school classes, each professor has their favorite set of minutia upon which they like to test students. There is no way that either Apollo's USMLE Audio Reviews or First Aid could read the minds of the professors at every med school - so you are stuck with your notes or videos or audios from your classes as an essential resource to pass those exams.
