Mobility of tooth

Increased mobility can be observed on radiographs:
  1. There is a widening of the PDL space without vertical or angular bone resorption bone resorption.
  2. There is no increased probing depth of the periodontal pocket.
Forces applied in one direction on the tooth give a widening of the PDL at the bone margin at the other side and in the apical area on the same side of the force.

Glass ionomer cements

It adheres chemically to enamel and dentine. Since it links ot calcium ions in the dental tissues, the bond to enamel is stronger than the bond to dentine because enamel is more highly mineralized.

Hybrid composites

  • A blend of both conventional glass or quartz particles together with some submicron particulate silica.
  • A total filler content of 80-90% by weight can be acheived.
  • The principal particle size is in the 0.1 to 1 micron range which is mainly minifiller.

Derivatives of micro-filled composites

  • The composites with filler particles not exceeding 0.6-1μm size(minifillers)
  • The use of a filler size below that of visible light wavelength enables the material to maintain a better shine, as the exposed filler edges will not readily reflect light.
  • Moreover, the larger filler particle size will also give better strength and mechanical properties than micro-filled materials.

Micro-filled composites

  • They contain silica particles with a mean diameter of only 0.04μm(microfillers).
  • They have a prepolymerized blocks of resin containing a higher filler loading of silica.
  • These blocks are ground up to give particles of resin-silica which were then mixed into more resin to form pastes of appropriate viscosity for clinical use.

Conventional composites

  1. They contain 60-80% by weight of quartz or glass particles of sizes ranging from 1 to 50μm(Macrofiller).
  2. The filler particles are coated with a silane coupling agentto enhance bonding between the filler and the resin matrix.
  3. The more modern materials contain larger quantities of smaller particles (1-5μm:midifillers) so that they can be much more easier to smooth and polish.

Composition composites

All composites are a mixture of a resin and filler. The two commonly used resins are either BIS GMA or urethane dimethacrylate.

Resin
BIS GMA resin
It is made by a reaction between bisphenol A and glycidylmethyacrylate. This resin is a very thick consistency at room temperature.
So, a diluent monomer TEGMA(triethylene glycol dimethacrylate) to reduce the viscosity and allow the filler particles to be added to the resin.

All composites are quite hydrophobic...

Resin composite

It is simply shortened to 'composite'.
Enamel
Although they have tooth-colored, they have special design on anterior teeth for esthetic and posterior teeth for strength and abrasion resistance.
Both of cases of the material are attached physically to the enamel by means of the acid-etch technique.
Moreover, composites is strong in thin sections when attached to enamel. The enamel margins can be bevelled, prepared with cavosurface angle of 90 degrees, or undercut.

Adhesion of the material to the tooth

Adhesion
  1. Adhesion to tooth substances is desirable, so that the filling may give some support to the tooth and be retained within it.
  2. Any adhesive material should minimize the gap between restoration and tooth so that it cannot produce microleakage to the bacteria potentially irritating to the pulp.
  3. Bonding of the restorative material may make it unnecessary to produce retentive features in a cavity.
