I have a few questions about NBME 11 concepts: <br />
<br />
1)&quot;An intravenous bolus of gonadotropin-releasing hormone induces a marked increase in serum luteinizing hormone concentration 1 hour later.&quot; What is this implying? I realize it is a defect in central system but I couldn't tell whether the defect was hyperGnRH or hyperLH or their hypo counterparts. (Question was about precocious puberty in a female). Can someone explain how this bolus procedure works and what its results imply? <br />
<br />
2) The melanoma that regresses is due to Cytotoxic T cells. However, NK cells also play a role, correct? If so, then why are these answer choices incorrect:<br />
Antibody dependent cellular rxn<br />
Antibody mediated cellular dysfunction<br />
<br />
3) The pneumothorax question: <br />
25 yo man comes in with SOB and chest pain during exercise. Has asthma and major depressive disorder...tempt and bp normal, RR=30, Osat =93%. No murmurs or increased JVP (Lab: Hgb=13, pH=7.46, pCO2=26, PO2=60). A chest x-ray is shown (sorry don't have this on me). What pul finding most likely?<br />
- Decreased breath sounds -<br />
If anyone still has the chest x ray - the trachea is deviated away from the lung that is pneumothorax'ed - that indicates tension and not spontaneou, however, his clinical vignette indicates that this is a spontaneous pneumothorax. Can someone please explain this?<br />

My apologies for not posting the questions/x ray. To those who plan to take NBME exams online, you should definitely make note of the questions you weren't sure about so that you can research it after your exam. Extended feedback, imo, is a total waste!<br />

The target date for reporting Step 1 scores for most examinees testing the week of May 13th through late June will be Wednesday, July 10, 2013.  For examinees whose circumstances require that they receive Step 1 scores before July 10, it is recommended that they take Step 1 no later than May 10, 2013.&quot;<br />

So what exactly does this mean? Are they gathering data from test takers in order to make a curve for the new test pool items? Are they just introducing new test forms or questions? <br />
<br />
Lots of things are left to wonder about why exactly they needed to hold scores for a month.</div>
