so im beginning to do some research for a few dental schools and i am a little confused about letters of evaluation. I need to ask 3 profs for letters but i cant provide them with the matching form that they need to attach until June 1st. Should i inform them of this so they are aware that they need this form before they send it.

And if they decide to do it via email, they have to write it out in word and then when i get access to the application of june 1 then i can enter their email somewhere in the application and its supposed to send them to evaluators portal where they can then submit it?

Someone please help me with this.

ps. you also need the street adress of the evaluator, how do you ask for this?
