LORs for St. George's

I was just wondering if the individuals giving an applicant a LOR for their St. George's University application need to fill out a recommendation form or simply send in their letters in an envelope to SGU. I couldn't find any additional forms on the SGU website. Also, if you are applying online, where would you send these letters, or any other supporting materials. Thanks.

back last spring my LOR writers just wrote a letter and mailed it in. the address to mail all materials to is:

c/o The North American Correspondent
