With your credentials and experience, it looks like you already know what you're doing. But for your list, I am only familiar with UVA and I'm pretty sure you'd get an interview from there.

Is this a good balance of schools?
You should be fine. If you are trying to cut down on the list, maybe get rid of RFU, OHSU (OOS), Washington (OOS), and Brown (a lot of 7-year BA/MD students.)

Curious whom you worked with while you were at UCSF...I did research there for a bit in the bioinformatics/biophysics area. PM me if you don't feel confident talking about it in open.

I would remove Brown, unless you're really set on it for some reason. SLU is also kind of random.

Other than that, you're applying to all the top 20 except Cornell, Pitt, and Vanderbilt. Any rhyme or reason to leaving those out?

I do think you will find a place at one of your "match" schools, assuming you're a reasonable person in-person.
