Default Questionnaires for Noncommunicable disease Study?

Greetings! We're developing a public health research study on hypertension risk factors in urban poor communities. We want to use self-administered questionnaires, and a're looking at the WHO's STEPS survey as a starting point. unfortunately, it's quite long and does not cover some important risk factors like salt intake

What are some good validated questionnaires for hypertension risk factors or heart disease or non-communicable risk factor diseases in general? We would greatly appreciate any input. Thank you very much!
