ok, you gotta explain this one to me, i thought that people who consumed unpasturized milk were at risk for brucelliosis (sp?) or even listeria.

Remember endogenous NAD+ (related to niacin) is generated from trp catabolism. If dietary trp resorption is impaired (Hartnup's dz) or if trp is being shunt toward other catabolites (Carcinoid syndrome --> xs 5-HT), then you'll get sxs of niacin deficiency.

**watch out for G6PD, toxic psychosis,ocular toxicity* commonly used with
tissue schizonticide Primaquine because of liver latent P.ovale and vivax,these forms could absolutely be seen in the U.S.

Sorry, this is wrong. There is more B6 in goats milk than cows milk. Goats milk is deficient in B12/folate, most notably.

eye/visual probs (vit A def)
hypocalcemia/bone prbs (vit D)
bleeding issues (vit K)
all kinds of nerve damage incl retinopathy, muscular degeneration, INCREACE IN LIPOFUSCIN CAUSING BROWN BOWEL SYNDROME
and hemolytic anemia and edema (all vit E)

The correct answer is B6 and Folate deficiency.

The Folate deficiency from Goat's milk could lead to Megaloblastic Anemia. This is known as Goat's Milk Anemia.

Take a look at Table 4, it shows that folate levels and B6 have much lower levels in goat's milk compared to cow's milk.

B12 and maybe folate are the key. I believe B6 is low yield, whereas if you see an infant on goats milk, 99/100 will have megaloblastic anemia.

Nobody can explain why it is a R-sided pleural effusion, either!! Technically it is CA, but it is 100% responsive to radiation.
 
Given by my Kaplan Path professor. Which one has more Folate, Beer or Goat's milk? The answer is beer.

So the best way to get dietary folate for Pregnant mothers is Beer!!!!

Wouldn't a diet with zero fat intake also lead to deficiencies in Vitamins DEK too...i.e. deficiencies in all fat soluble vitamins?

Well I think Vitamin E and D would also quickly deficient but Vitamin K is produced by our intestinal "normal" flora, this is the basis of infant hemorrhagic disorder. So I would assume you might become deficient of vitamin K much later on if you were on a no fat diet.

The key is the absorption. Even though it is part of the flora, the vitamin cant be absorbed without fat.

I dont like this...asthma is a reactive airway disease, but has an obstructive pattern of disease. This is not a classic restrictive disease like sarcoidosis or interstitial fibrosis, but, rather, is due to air trapping and increased compliance (similar to emphysema). I just had a question for Sporotrix on Q-bank...DOC is Itraconazole. I thought KOH also (as per micro ridic simple), but it wasn't among the answer choices :confused:
 
You are correct......S.schenkii can be treated with either itraconazole or potassium iodide :thumbup: :thumbup:

Careful here. "crew cut" can also be a presentation of sickle cell anemia. I had a question about this. Don't get caught on this Buzzword!

Thanks...I too had a question on this. Sickle cell was one of the answer choices and Beta Thal wasn't. But you are right on!
 
On the jugular venous pulse

c-wave-S1, tricuspid closed,bulging of tricuspic iinto atrium (twisting of the
ventricles after AV valve opens)

v-wave systole, *filling of R.atrium, terminates when tricuspid valve opens,
beginning of S2

I think you meant "twisting of the ventricles after the aortic and pulmonic valves open" The AV valves are closed at this point as this is the begining of systole and the S1 sound you mention is the closing of the AV valves.

Question.. What vitamin would be missing in a person who is a vegetarian and DOESNT eat milk and eggs and one that DOES eat milk & eggs. I had this question and I figured it was either B12 of Vitamin D. IN FA it lists both as sources of both.. WTF?

B12 is only found in animal products, I believe.
 
vitamin A and vitamin D can have toxic overdose because they are fat soluble (can be stored) and have steroid-receptor mechanisms (zinc fingers). vit E 'toxicity' is via warfarin potentiation

vitamin A tox --> Arthralgia, Allopecia, Aching head (cerebral edema), Aching throat, and Apathy (well..fatigue, but i was on such a roll), and cirrhosis

vitamin D tox causes metastatic calcifications--can be associated with sarcoidosis. made in 3 steps at the skin --> liver --> kidney (PTH-25)

vitamin A toxicity doesn't bother the eyes because here it works at the outer rod segment via inhibitting cGMP (rhodopsin). a common source is a yummy yum fatty fat fat animal liver

I couldn't believe it. I had a usmle question about some guy who had some sort of malabsorption situation ( I can't remember if it was drugs he was taking--like anti cholesterol or if it was his diet)

AND LO AND BEHOLD

one of the answer choices was loss of night vision. I chose it.

None of the other answers made sense.
