And is it really impossible to get a fellowship in Plastics after general surgery if you are an IMG (St. Georges).

what exactly is the question?

You've answered the first part of your question when you discuss fellowship.

Second part I don't really know about. Your best bet is to get into a good GS categorical program. Do well and you'll be on equal footing as everyone else.
