For a paper in which you are listed as an equally contributing co-first author, but you're listed 2nd (alphabetically), is it acceptable to enter yourself first on the ERAS authorship line to make sure that PDs understand your contribution to the paper?

Yes, when you're a co-first author, each of you lists your own name first on your own CV, noting that the other author is a co-first author.

If the publication officially lists co-first authorship, for example:

then on your CV you would replicate the citation. It would not be convention, using the first example above, for Liu to reverse the order of their names when listing the publication in her (his?) CV. It would, however, be appropriate for Liu in her CV to include two asterisks (*) or hash marks (#) next to the names of Zhang and Liu and then at the end of the citation write something like (#, authorship equally shared).

This is the appropriate notation as described, for example, on the Nature web site:

That notation is accurate, if citing the article. But I have a formal document on CV standards for a particular academic institution, that says it should be done as I noted above.

Just bumping this thread as I have the exact same question and cannot find any other information. Does anyone else have an opinion on how to go about this on ERAS where, unlike a regular word document CV, you cannot list yourself first and also denote that the second name is the co-author?

I appreciate any insight or suggestions.

When I entered the authors on my paper, I put a * next to myself and the other co-author and then at the end of the list of the authors, I put *co-first author (I guess maybe I'll change it to equally contributed to the manuscript).

Like this:

Robbins, T.*, Edwards, M.*, Jones, W., Smith, R. *co-first author

I think that clarifies it well enough and I don't want to put my name first if it won't be like that on the publication, because if someone believes it's incorrect, then I don't want them to think I'm falsifying information.

I also have a publication on which I was co-author, but on the actual publication my name appears second. On the paper, it notes that the first two authors contributed equally to the work. On my ERAS, I listed the authors just as they appear on the paper, with my name second.... did I short-change myself by doing this? I did not comment at all on the fact that I was co-first-author.... I didn't even think to do that until I saw this post... and now it is too late! aggh! what can I do now?

Not a big deal I would say. Second co first author isn't really much better than second author.
