Default International student, 31Q, 3.95 cGPA, 3.92 sGPA

I am an international student studying here in the US. Just got my mcat scores and received an 11/8/12 for a total of 31Q. I have done 50+ hours shadowing, 100+ hours volunteering, and 2 years research with one publication as a second author in a respectable journal. I may be interested in doing MD/PhD, and I am also wondering what my chances are for some of the upper tier schools. Any input would be appreciated!

I think as an international applicant your MCAT score, especially the VR section is too low for
"upper tier" schools.

Yeah, that is what is worrying me. Would it be fine for mid-tier schools?

OP please make sure you understand the impact of your immigration status on medical school admissions. Are you a permanent resident?

Your visa status will greatly limit your school list and your MCAT will hold you back from the top tiers. MSTP is out unless you retake.

Do some digging to find which schools are receptive to international students. Also, remember that you will have to come up with a lot of money prior to matriculation.

Very few med schools will accept a person not on a permanent visa.

Your first task is to find out which med schools accept an F-1 visa. That'll narrow your choices down significantly, and then look at their averages.

There's no such thing as low/mid/high tier for you. You are limited by a small set of medical schools because of your visa status.
