Default Question about letters of references

Do you guys ask your professors to include the course names and course codes (i.e. BIOL 201 - Biochemistry) of all the classes you've taken with them in their letters? I'm assuming dental schools would want to know which classes on my transcript I've taken with them.

For the dentist reference, do you tell him/her to include the number of hours and time period of shadowing?
Chances are pretty good you do not need to "tell" a professor/dentist what needs to be included in an lor unless, there it is specified by a school in question, in which case, it would be wiser if you asked them.

Hey I had a question about LOR's too... I emailed an old professor and he said he would be fine with writing me a letter but then asked me
"who it's going to (with their address), and if they have a form to fill out or just a letter from me, and the deadline."

Obviously it's for dental schools but who is it going to exactly? I don't plan on applying till next year but thought I should get on the ball and get the LOR's ready from now. How exactly does one get an LOR ? I always thought I would come by and just pick up an envelope with their letter and then fax it over whenever I apply. And deadline for what?

It's going to the American Dental Association. You don't fax it. The professors themselves mail it or submit electronically via the AADSAS website. The deadline is June through February every year.
