I think it's a good idea to seriously consider waiting a year. Your application just isnt quite ready. 50 hours of hospital volunteering for clinical experience. No shadowing. No non clinical service. These are pretty big weaknesses. You are judged by what youve already done when you submit an app not what youll do during a gap year.
These ECs in particular wont work for top schools if that's what you are targeting. South Carolina is a fantastic state for MD admission but even with stats this high the areas your ECs are lacking in will make things dicey even despite being in such a lucky state with multiple lenient state schools. Address your weaknesses, apply in a year, and you wont have problems getting an acceptance. Keep in mind only 4% of South Carolina applicants(30 of them) matriculated OOS last year. Those 30 likely got into top 20 schools or had other unique circumstances. In other words, really applying to any OOS school that isnt a high end one is likely not worthwhile.
