I have had interviews that went like the one you described where everything felt right and I didn't get the job and others where I felt like I bombed it and got the job. It is inherently an unpredictable process and this uncertainty increases anxiety. When hiring people I tried to contact people as quickly as possible with a yes or no and be as transparent about the process as possible to help alleviate the anxiety of the waiting part.
As far as who to follow up with and when, that is unpredictable too! You just have to use your best judgement as you were the one there. I usually make at least one follow up call after a few days and it would usually be to whoever is the decision-maker.
