another pain fellowship question

Excuse my ignoranace, but I am just newly interested in obtaining a pain fellowship spot. It sounds like pain fellowships or open to applications from any specialty. How are applications handled - does one apply to each program separately? It sounds like applications are have already started for 2008? And it sounds like most programs fill - if they do not is there a way to find out open positions?
