As NRMP Match Report said:

Physician (R) positions are PGY-2 positions starting in the year of the Match that are reserved for applicants who have had prior graduate medicaleducation.

Physician positions are not available to senior medical students. In previous Results and Data Reports, the numbers of R positions were small and they were included in the categorical position counts. In this report, R positions are listed separately for the entire five-year period.

does it refer to those guys did not match after graduation?

Physician (R) positions are reserved for people who have completed at least an intern year (medicine/surgery/TY). This could be for people who didn't match into radiation oncology but pursued an internship anyway, or for those who have started and/or completed another residency.
