April 30th deadline, MDPhD waitlist

Hey so I have been accepted to a few MD programs and am on the waitlist for my number one MDPhD. I got accepted to their MD program, just waitlisted for their MDPhD. However, with the April 30th deadline coming I have to narrow those decisions. I could prefer a different MD over their MD program alone. If I withdraw from their MD acceptance, does that rule out my chances at their MDPhD offer? Or does that acceptance negate my decline?
