If you could choose from any MHA program in the US, which would you consider to be the best for landing fellowships with health systems in California or Texas?

UMich? Hopkins? Columbia/Cornell? Yale? UCLA? USC?

Why?

I notice there are no Texas schools on your list. Trinity in San Antonio. University of Houston-Clear Lake.

As a Texan that has an MHA (from neither of the above schools), these were the institutions doing most of the fellowship placements when I graduated. That was in the dark ages of the 90s, though.

All the programs you have mentioned above are highly ranked nationally, I would imagine they would have excellent fellowship placement regardless of region.
 
I may be biased because I just accepted an offer from Trinity, but they are hands down the king of Texas health care administration. Check out the alumni list or PM me and I can give you some more resources to look into.
