When I go to the website and verified my name and intern license number, I realized that they misspelled my name. On this form and the board's website (http://www.pharmacy.ca.gov/forms/duplicate_license_fee.pdf), it states that if you failed to receive your license, the reissue fee is waived. Well, I did not failed to receive my license. It is just that my license was issued recently, and the paper copy has not arrived yet. So, do I still have to attach a $35 check along with the application form? Thanks
