I just got my MCAT score, 32 overall, 10PS/11V/11BS. I'm still thinking planning on applying this cycle.

I attend a small liberal arts school.
-near perfect gpa, phi beta kappa, deans list etc.
-neuroscience research for 3 semesters (and continuing)
- Clinical research internship at Johns Hopkins, clinical research internship at Boston Children's hospital
- 200 hrs Hospice volunteering
- 20 hrs shadowing
- variety of non-clinical com. serv - 50 hrs.
- president of sorority
- Lab assistant and tutor
- Part time job
- Good LORs

I made this list before the MCAT. What do you guys think I can change/get rid of/ add? I'm from massachusetts so that kind of limits me in state wise.

Take out non-Mass. public schools. They barely accept any OOS

You should DEFINITELY apply this cycle. Unless you'll only be happy at one of the "top" schools, I think you'll have an excellent chance at being accepted. Great GPA (though I don't know much about your school, but that's hopefully beside the point), solid ECs, and an above average matriculant MCAT.

That being said, the MCAT will probably hold you back from a lot of those. I would suggest dropping UCSF, any publics that aren't IS, Hopkins, and Cornell. Add more schools that have MCAT averages around 33-34, regardless of their GPA. Your ECs are okay, but not excellent enough to compensate for your (reactive to your GPA) low MCAT. Sorry :(

Unless there's something significant about your app that you're not telling us, you're not getting into UCSF. Your MCAT isn't competitive and you're OOS.
 
I would replace some of those higher tier schools with even other reaches that are not ridiculous reaches. That is unless you have like 10 publications you forgot to mention.
 
Concur that your MCAT is well below avg for the UC system, but definitely go for state schools adjacent to MA. Suggest forgoing other OOS public schools except for 1-2 you really are interested in, and have a track record of taking OOS students.

I know UCSF is ridiculous, it's been my dream school for a long time that's mostly why it's there. I love the west coast it's hard to give up on all of the schools out there being OOS, do you think any of them are in reach?

I definitely think it's too top heavy. I'll take out colorado, UCSF, and NYU (?) and add quinnipiac and Uconn, I have a strong aversion to Drexel. Maybe add Maryland (I don't know how they are with OOS?) and Duke?

Loma Linda is a long way from the bay but that's the one MD school that might work for you in CA. You do have to be religious. Duke, Hopkins, Darthmouth, Cornell!?
 
One of my LORs is coming from a well-known MD/clinical researcher at Hopkins. Duke was Goro's suggestion. I know that Dartmouth and Cornell are reaches obviously but are they a waste of money?

I don't think Dartmouth would be but Cornell probably.
