Deciding between OMM ans choice and medical treatment on the COMLEX

How does one differentiate between these two during an exam when we are 50-50 , anyone have any insight or advice? What answer are they looking for? Or will it always be clear cut?

The choice is always OMT unless there is a contraindication like trauma, infection, bleeding, etc. usually OMT is not 1st line management. They usually ask what an alternative treatment would be or what else you could do.
