Right, but he/she would still know miles beyond his midlevel counterpart ( 3rd year nursing student? idk )

Really? when a patient crashes at your hospital? you don't get the physician - you handle all this yourself? when an M.I occurs, a baby has RDS, there is postpartum hemorrhage, or a patient crashes during surg, your NP's, DNPs' and CRNA's handle this all themself? I should meet you and your crew of super nurses / PA's. Impressive.

You would be surprised how many "crashes" I've taken care of in my 37 yr career, even ordering meds, fighting one doctor and insisting that a patient be transferred to ICU etc. The attending have always covered me later by signing orders.

I just like to spar a little when I have time, and when you guys need straightening out. Actually I have nothing against physicians except for a few dipsticks I've run across. I never plan to walk in your shoes but I have walked with you since I first stepped in a hospital in 1970. No, I do not know exactly what your experience is but I'm no dummy. I do applaud you guys for going so far into debt getting into our current medical system which is rapidly going downhill. Hopefully you can make some changes. Nursing is screwed up also BTW.

I don't care for all of his policies but respect the position.

I respect the position, not the person. For just one example, anyone who has the gall to accept a Nobel Peace Prize for what "he's going to do" ....well I just have no words.

Nursing, in spite of a powerful lobby, just can't get it together, IMO. There are so many entry levels and direct entry programs that I can't keep up with them. NP programs need some revision and more clinical hours, but it doesn't need to follow the PA curriculum. The DNP was supposed to add more clinical hours but turned into some kind of combo MPH/MBA degree, which is alright in itself, but not what myself and others wanted. We,ve already had 2 doctoral programs in the past designed to have more of a clinical focus, the Nursing Doctorate and the Doctor of Nursing Science.

What do I do? I recently became a Psych NP which goes great with my shamanic practice, although it is less effective. What do you think about that? :eek:

1.)It would be seen as a slap in the face of the Nobel committee for him to reject that award. Remember he did donate the cash from it to charity.
2.)You should be respectable to the person that earned that right to be in that position. It would seem as a NP with so many people that disrespect you due to that reason you could possibly correlate that to your personal life somewhat. Again, although you don't agree with his policies you should be respectable to his position and vote him out in 2012.
3.)If you don't respect Obama did you respect Bush or Clinton?

I would be curious as to how the coursework for nursing/DNP compare to this...anyone know?

1.)Well about President Obama-Him becoming the first African American President deserves a tad of respect. He also seems to put his family first which deserves respect imho. His policies are more than what the Repubs have tried to do-Its much easier to say NO then try to reach a compromise. Your overall generalizations pretty much disgust me sir. Nothing positive all at except name calling. I am assuming you hadn't lived much have you? How old are you?

You would be right. He is an AA from Emory. Go figure(rolls eyes). Also how can an AA be the Chief Anesthetist??? I am actually debating about going into Gas vs. EM vs. FM. I hope I don't work with anyone with that Elitist attitude. Irks me to hear anyone calling anyone a Buffoon but again from a MLP saying it, it just appears to be worse since so many people don't believe what we are doing is the most appopriate.

Not to nitpick vocabulary, but I wonder if jwk meant to say, "Clinton was a buffoon, yet still a canny politician." The sentence makes more sense using that word. "Cagey" just doesn't fit.

I just don't understand how someone can call Clinton (or anyone for that matter) a buffoon while in the same sentence describe Bush (assuming he was talking about Jr.) as "fine." Politics aside, there were times I seriously questioned whether the man could read.

You must be the kind of person that thinks every black Republican is an Uncle Tom. Sorry, I see nothing positive about a corrupt Chicago politician sitting in the Oval Office.

If he was interested in compromise, he wouldn't have rammed health care "reform" down our throats. The Democrats had to break their own Congressional Rules and system of checks and balances between the two houses of Congress to get it passed. They were'nt interested in compromise or they would have listened to at least SOME of the initiatives proposed by the Republicans. They didn't. They were hell bent on passing what they wanted and to hell with anyone who got in their way.

Of course I'm a Republican. Do I agree with everything the Republican Party does? No. Do I agree with everything that W did? No. But consider the alternative - it scares the crap out of me to think what might have happened had Gore been the President instead of Bush.

I'm in my 50's, and I've been in practice for 30 years, so I think I might have just a little real-world experience and perspective than you do. You're welcome to your opinions, and I'll keep mine, thank you very much - and like it or not, agree or not, Clinton was a buffoon IMHO.

I said that MLP comment due to many people don't want us around in the first place and we are not nearly as respected as we should be so although it was a stretch of an analogy I guess you were not able to put it together due to me stretching too far.

Also how about those magical WMD that were in Iraq still waiting for those to be found...and speaking on corruption how about good ole Dick C.....and I agree term 1 we needed Bush in the WH but he shouldn't have been there for the second term....

Do you have any solutions? Also if you think the status quo is sooo bad you can run for public office and change things.

So you think the President automatically deserves my respect? Sorry, not gonna happen. Respect is earned, not bestowed by title. Obama has done nothing to earn my respect. Now - declining the Nobel Peace Prize because he had clearly done nothing to warrant receiving it would have been an act worthy of my respect. But alas, he missed that opportunity. So - if you love him SOOOOOOOOO much, explain to me - what had he DONE at the time the announcement was made that made him worthy?

McCain most certainly is worthy of respect, given his long history of service to this country (literally decades more than Obama). Palin wasn't the best choice, but even now, she's a force to contend with whether you like her or not. I'd still take her over Biden any day. Is there a Democrat I would vote for? Of course not. I'm all for healthcare reform - I just don't like it as defined by Democrats, and I don't like the way it was rammed down our throats. I'll admit - there are a few parts I do like, such as being able to leave your kids on your insurance till they're age 26, but hey, guess what - I pay for it. It doesn't come free. Your solution is what, the government paying for it, a single payor system? Laughable. Medicare/Medicaid don't cover actual costs now, and Obamacare did nothing to deal with that. The cost is still born by those of us who are having to pay for it ourselves AND have to cover the cost for those who can't, or more importantly, WON'T pay for it themselves. The Canadians love their system - until they really need it. Are you ready for rationed care?

I have no problem paying taxes insofar as it helps those who CAN'T help themselves. My problem is paying taxes to take care of those who WON'T help themselves and EXPECT others to do it for them. There's a reason it's called "entitlement".

5.)I wish you would come to other parts of the South and teach that to some of these Level 1/2/3 Hospitals. Its quite sad to watch someone(Physician) willing to let someone die for the almighty buck and fear of getting sued. Let me ask you-Will these doctors accept patients from outside hospitals for transfer? Or only take care of those people that walk through their front doors. If so that is totally different beast from a standard of care issue.

Surely, DNPs would introduce themselves as "Dr. Smith," and 99% of patients would not know the difference between an MD/ DO and a DNP. This is false advertising! Patients will be expecting an MD (not even a DO!) to be treating them and will not be able to exercise appropriate autonomy under such circumstances.

An RN versus an NP is a useful distinction. A DNP blurs the line too far and ought to be rigorously regulated to prevent the erosion of our professional boundaries. We sould go the European route and legislate who is allowed to call themselves "Doctor," as this is a dangerous road to travel down.

1.)Disagree with you on a few points on an overall good post. The first one being that nurses(well seasoned nurses are a God send a new one six months out of school not so much. (Same goes for ALL healthcare providers) know patients better than Residents/Attending/etc... This is a misnomer. There have been a buttload of times if I would have let the nurses history dictate my plan of care I would have sent a critical patient home(my oh she just started her period today and I want to triage her home and you don't need to see her-Sent from a local clinic due to a prolonged period and a critical H/H and needed transfusing. Another one is a patient that "just bumped her head"-patient ended up having a bleed->I have had that scenario happen TWICE in TWO different hospitals from RN's(again the younger ones) that supposedly knew the patient better than I did.
2.)Patients expect both MD/DO. I cannot speak for you but before I went to medical school, I preferred my Docs to be D.O.'s when I had the choice due to them listening to me. Most of my M.D.'s kind of blew me because they had a full patient docket and time is money. Also D.O.=M.D. btw due to those nice learning experiences called residency.

I agree with you, Nurses are the face of any hospital. NPs however, who operate in a mid-level capacity have about as much contact time with each patient, as every doctor,resident, intern etc...

As you said it would be good to distinguish who is allowed to call themselves doctor in a medical setting, in a way we have already done this. The term Physician is highly regulated, as only an MD, or a DO can legally address themselves as physicians without any pre/post physician titles. Many hospitals have switched to large banners underneath the hospital badges with a different color for each profession along with PHYSICIAN, NURSE, PHYS. ASST., ENVIRONMENTAL, TECH, ETC... I think the system is great, you can clearly see/know who/what everyone is about 15ft away. Especially these days when everyone is a "doctor" and even house keeping staff are dressed as doctors, (scrubs and long white coats).

I know that in medicine we have a tradition that only a physician ie: resident or attending can wear long white coats, and students can only wear short coats, to easily distinguish who is a student, versus who is an attending. Now all of a sudden it seems anyone who is not a physician can just wear a long white coat... just more people not secure in their field.

Be happy with the degree that you got, and do not try to represent yourself as something higher or greater, if you wanted to be a NP, its a good job prospect and is a noble career, but do not ever try to represent yourself as a doctor, understand you are an NP, and didnt go to medical school, and you are selling your own degree short if you were to advertise yourself as something you arent.

Components of the bill
- Prevents insurance companies from denying coverage to individuals/family members with pre-existing health conditions; a temporary plan is being developed to cover high-risk individuals with pre-existing conditions until the full reforms go into effect in 2014
- Prevents insurance companies from placing lifetime limits on benefits
- Bans “rescission” so insurance companies can’t cancel coverage if individuals keep their policies current or if they become ill
- An individual’s out-of-pocket healthcare expenses are capped
- Closes the “donut hole” (Part D) for Medicare prescription drug coverage (under Bush, Medicare helped pay for drugs up to $2,600 and above $4,550, but individuals had to pay 100% of the costs in between these amounts); now Medicare helps cover costs irrespective of the amount – seniors will now pay only 25% of drug costs up to $4,550 and only 5% of drug costs above that amount
- In 2010, an emergency provision will offer seniors a $250 rebate on the costs incurred within the “donut hole”
- Individuals living at or below the poverty line were eligible for healthcare under Medicaid, but by 2014 individuals/families living slightly above (making up to $14,404/$29,327) the poverty line will also be eligible for benefits
- Individuals/families making less than $43,320/$88,200 per year will qualify for government subsidies to help purchase health insurance
- All individuals must have health insurance or face a government fine; all large (over 50 employees) employers must offer health insurance to employees or pay a fine
- Small businesses can get a tax credit if they offer health care
- There are hardship exemptions if individuals can’t afford health insurance
- Families can keep their children in college on their plans through age 26
- Promotes health insurance “exchanges” so consumers can buy “wholesale”
- Creates consumer assistance offices to help consumers file complaints or appeal decisions from insurance companies; beginning in 2011, insurance companies can no longer make excessive rate hikes without justification and approval, and those doing so may be barred from participating in new health insurance exchanges
Funding sources:
- Large employers (over 50 workers) that don’t offer health benefits will be charged a $2,000/worker fee; if the employer offers coverage but employees instead purchase federally subsidized insurance the fee is $3,000/worker receiving federal subsidies or $750/worker (whichever is lower)
- Annual fees on pharmaceutical companies ($27 billion), health insurance companies ($60 billion), and medical device-makers ($20 billion)
- Annual penalties on individuals who do not have health insurance (up to a maximum of $695/person)
- Increase in the Medicare payroll tax from 1.45% to 2.35% for individuals making $200,000+ and families making $250,000+
- 3.8% tax on unearned income for millionaires
- Insurance companies will be subject to a tax on each high-end insurance plan (so-called “Cadillac” plans) they offer
Miscellaneous:
- Illegal immigrants are not eligible for insurance or subsidies
- By Executive Order, such federal funding can’t be used for abortion
- The federal government will assist states by covering all of the increased expenses of expanding Medicaid coverage (90% of costs after 2020)
