Are there any historical examples of specific procedures trending upwards in insurance coverage? I'm curious about the factors that go into this.
I mean, nobody really shops for coverage based on specific procedures, right...so the providers can't really leverage their potential patients to lobby much (unless it's a common procedure people are worried about). So does it simply come down to the providers' ability to negotiate collectively with 3rd party payers? If so, do the national and state professional organizations encourage or facilitate the providers to do just that and not break rank?
