ERAS cover letter

how do I obtain a cover letter for one of my LOR? My department head is saying that before writing a letter he needs a cover sheet but I have absolutely no idea how to get this

You print it off ERAS

so I have to "add a LOR" and plug in details and then it'll show me cover letter?
