I am wondering which of the Direct Acting Oral Anticoagulants (Rivaroxaban, Dabigatran, Apixiban) are typically included on a hospital formulary?

I am a P3 and have to make a recommendation for a class assignment, however, without any clinical experience through APPE, I am not sure what to recommend and key things to factor into the decision. So any comments would be greatly appreciated, especially a brief explanation why particular agents were included or excluded from your formulary.
