futile care vs. advance directive

Why is it OK to deny a procedure based on the thinking of "futile care" and then we have to put someone on ventilator for good if ventilation is what the pt wanted in the living will? In the later case, would you not deny the ventilator also based on the irreversibility of the disease and you know it's not going to make any difference in the outcome?
