help me plz with residency training in GERMANY

Hi all
I'm Iranian medical graduate and graduated in 2013.
I want to apply for surgery residency in Germany.
so far i have only learned Deutsch. I'm now at B2 level.
The problem is what should i do now :D
i don't really know how should i get position since i want a paid one.
i don't know which cities are better. should i start from small towns ?
can some one please help me and give a step by step processes?
another question is i heart it is so much better if i apply from Germany. is that true? and so should i start with an observer position in there and while i'm there, apply for residency?

please help me i'm so much confused and the path is wirklich unclear for me :(

You should probably contact the hospitals that are advertising positions. In nearly all states B2 level German is required. I think only in Hessen is C1 needed. I have met quite a few international med graduates that have started their specialisations in Germany. You'll probably find it difficult finding a place in the bigger cities (especially Berlin) though. Even my girlfriend, who is a german graduate is finding it hard to get a position in Berlin... Whether your education is recognised here or not is another question.. Good luck!
