Can somebody explain why Heparin causes hematomas if given IM?

Check with your state BOP regarding what you can and cannot do. I'm in California and routinely make this sort of change after a brief discussion with the prescriber.

It's based on arcane statements the DEA made about changing things on CII scripts. The drug name is one thing they either forbade changing, or left up to states to decide whether or not it is ok. And the APAP content differs for some of these hydrocodone products, so they aren't all interchangeable.
