I just got an acception from a neurology program (with great reputation) that only sponsor J1 visa. But I have not made up my mind whether I should accept that. How difficult is it to find a J1 waiver job after neurology residency?

I don't know, BUT:
I think it might depend on whether or not you do a fellowship first, and how keen you are to work in that field. As a general neurologist, or as someone who doesn't mind working outside their 'fellowship field' it might be easier...

Another thing to consider: How interested are you in staying in academic medicine? Again, I'm not sure, but I suspect it is a lot harder to get involved with research on a waiver...
