Let's say I'm a practicing family physician and I see there is a phase III drug for diabetes that shows a lot of promise. Is there a way that I could go about enrolling my patients in the trial? Do companies usually ask doctors or do doctors usually ask companies? How does this work?

Most trials have their own recruitment strategy at large academic places or have well oiled (corporate) machines to get patients/participants enrolled. There are also some smaller trials that probably enroll from private practice, but this is rare.
If you have a patient who is interested in a trial, one can always find the trial and the contact info for it at: clinicaltrials.gov .
