How does a cardiothoracic surgeon make money in private practice?

Does the CT surgeon have people the come to them that just say I need surgery, or does the hospital like send patients to the CT surgeon when someone needs CT surgeon. With other practices such as family practice people come to the doctor for checkups, minor health problems, etc. But if someones having a heart attack they don't go to a private practice office they go to the hospital. Just how does a CT surgeon generate revenue for his practice.

They get referrals from/consulted by cards/pulm.

You are asking a very specific question, but what you really need to do is gain an understanding of how medicine works in general.....referrals, ER call, EMTALA, etc. None of your questions are specific to CT surgery, and you are unlikely to find someone in a surgery forum patient enough to explain this relatively remedial concept. they don't. you can't make a living in CT surgery in a private-practice model.
