With a 3.6/34, your best chances are theoretically at schools like:


Tufts, Einstein, Rochester,
UConn, Georgetown, Maryland, Stony Brook, Cincinnati, Penn State,
Albany, SUNY Downstate, Hofstra, Jefferson (I threw in a few you might not have considered),
with Drexel, Temple, and your state schools as good safeties.

As far as your reaches and super-reaches go, without a strong research component to your application, or at least above-and-beyond leadership, be prepared that they may take a pass. Your clinical experience as I perceive it, however, is superlative, and I hope it garners you some attention that your lowish BCPM might have lost you.

Not that I've noticed.
In fact, it actually seems like they tend to wait so that the people they accept are more likely to stay there. That is, people who wanted to go elsewhere withdraw, and they're left with those who're (supposedly) trying to stay in-state for school and practice.
