I'm an MS-3 at a DO school planning to apply to ACGME neurology residencies in the fall. Regarding IM prelim internship applications, how important is it to do a IM sub-internship early in 4th year and secure a LOR from that sub-I? I already completed 8 weeks of IM during 3rd year and have 2 LORs from those months. Thank you!

Not super important for a prelim. They know you're only applying to be there for a year and don't want to practice IM. However, it certainly doesn't hurt to have a strong IM LOR.

You have two IM letters already. As long as they are decent you should be good.

FWIW I matched to a top TY without any Medicine letters. I had surgery. They just want to see from your letters you are a hard worker and not a serial killer...
