What are my chances for MD vs MD/PhD?

Currently top30 private, junior
MCAT 520
GPA: 3.8

Research
4 yr research at one lab at top50 med school(2 publication, poster presentation at national conference)
2 summer research programs with poster presentations at home institution and top3 med school

Hospital Volunteering
ER 350hrs

Shadowing
Nursing home/rehab center 200hrs
Various surgeons (thoracic, vascular, ortho) 200hrs

EC/jobs
TA, tutoring, medical typist, club leaderships

GPA good, MCAT good, ECs look good but there's a lack of non-clinical volunteering.
Numbers wise, very competitive.
