If a patient warrants a breast examination are you allowed to do that with her informed consent? Would that be sexual misconduct since she technically has a mental illness? If so how do schizophrenic patients get screened for breast cancer then?

There's a difference between mental illness and mental (and legal) incompetence.

If the patient is capable of consenting to medical treatment and a breast examination is warranted, it's not "sexual misconduct".

Schizophrenics and anyone else with mental illness gets screened for breast cancer in the very same fashion as anyone else (not withstanding the disenfranchised for whom medical care is difficult to access).
