Other than providing faculty with CVs and Personal Statements for them to write letter of recommendations, what else is suggested to include?

When you ask them if they are willing, you can also ask them if anything else would help besides a CV / Personal Statement. You can also offer to have lunch or coffee with them.

If they agreed, I'd come back the next day or the day after and give them a folder with that information. I ended up included my transcript because one faculty asked for it.
