My stats:

1500 hrs clinical research
1000 hrs research, poster, pending pub, leadership position in lab
100 hrs hospital volunteer
Played musical instrument in college, performed concert
hobbies

GPA is low because i took ~18 units regularly, worked a lot/did research a lot but also lost focus sometimes

Is everything a crapshoot for me? should i even bother with the top 20 programs?

I don't know enough about admissions to suggest your stats are good or bad, but your volunteering looks a bit skinny. Great research though! Good luck!

With a GPA that's well below avg, I don't think you'll have much luck with any MD program, except perhaps the lowest of the low tiers. You definitely will not get into the top or medium 20. Unfortunately, MD programs get so many quality applicants that they can afford to ignore applicants like you.

You are a high risk applicant. It is very difficult to predict which of the schools on your list might have a screener willing to take a chance on you. A lot will depend on how the non-quantitative aspects of your application are received.
