1. Graduate Programs:
Please indicate one or two PhD programs of interest to you (one selection is required). This will help us to understand your areas of interest but will not commit you to those programs.

6. Required Essay: Greatest Challenge
The admissions committee is interested in gaining more insight into you as a person. Please describe a significant personal challenge you have faced, one which you feel has helped to shape you as a person. Examples may include a moral or ethical dilemma, a situation of personal adversity, or a hurdle in your life that you worked hard to overcome. Please include how you got through the experience and what you may have learned about yourself as a result.

Please limit your response to 1 page (about 3,500 characters), and leave a blank line between paragraphs. If the 3,500 character recommendation is exceeded, your essay WILL still be saved in its entirety.


7. MD/PHD Research Essay

The CWRU MSTP/CTSTP requires answers to two essay questions that are the same as the MD/PhD questions on the AMCAS application (the Research Essay and the MD-PhD Motivation Essay). If you indicated MD/PhD on the AMCAS application, and answered these questions on the AMCAS application, you need to indicate so in the answer to these questions here, and we will review your answers from the AMCAS MD/PhD questions. If you are satisfied with your responses in AMCAS, type "Please see AMCAS MD/PhD essays." in the fields below questions 7 and 8. If you did not indicate MD/PhD on the AMCAS application but desire to do so now, or if you wish to provide a modified answer to one or both of these questions, please provide answers to the following questions.

Please describe your significant research experiences. In your statement, please specify your research supervisor's name and affiliation, the duration of the experience, the nature of the problem studied, and your contributions to the project. If your research resulted in a publication on which you were an author, please provide the full citation.

Please describe any ideas or plans you have for PhD research or rotations, including any specific PhD mentors or PhD projects that you may have considered.

Please limit your response to 3 pages (about 10,000 characters), and leave a blank line between paragraphs. If the 10,000 character recommendation is exceeded, your essay WILL still be saved in its entirety.

8. MD/PhD Motivation Essay
In the field below, please state your reasons for wishing to pursue the combined MD/PhD degree.
