Measuring Patient Risk

Does anybody know of any systems used to measure patient risk that currently exist in the literature or are being used by hospitals?

My current thoughts are some sort of risk categorization algorithm that includes things such as chronic conditions, technology-assisted, missed visits, number of medications, etc.

We have a charter to measure coronary risk.... it takes in consideration different conditions, and there are different coloured risks according to every specific thing, and afterwards you can quickly have the cardiovascular risk in order to give the better treatment...just in case you need it, just ask me for it -
